Friendly Uruks kill each other during missions?

Ive had this happen a few times and im not sure why. The first time was a chase mission, i had 3 Captains (Mine) vs One enemy captian. For some reason two of the captains killed the third. They just betrayed him but stayed loyal to me. And then during a siege battle one of my Captains killed another one of my captains. why does this happen and how can i stop this?

Make sure there are no rivals in the same place, they will rage and go after one another.

also make sure no one has berzerk trait ( attacks ANYONE friend or foe )
also friendly fire after 10 hits they kill anyone in their way ;I

If a berzerker is enraged he can attack anyone near him, including allies (but I don't think a follower berzerker will attack you). Also certain hatreds like Enraged by Weakness/Cowards can cause captains to attack each other. And of course rivalries will do this as well, like others have said. I have not tested this, but i think it might be possible that accidental friendly fire can cause a blood brother to take revenge on a friendly captain as well.

Thank you all. i was unaware of whether or not rivals had anything to do with it. Due to the fact that Talion basically mind controls his Uruks. I wont let rivals fight with along side each other anymore. Once again, thank you.

If one orc kills another orc who has a blood brother, the living blood brother will attack the offending orc on sight regardless of being friendly or not

i had one time where two guys kept enraging eachother... i had to body block their swings to save the weaker one then dominate him and send him away. but yeah if you ever notice it happening just dominate one and tell him to retreat or give him other orders.
