Some achievements aren't working or I'm missing something

I passed all missions, killed Dr. Balanced, where is my achievement for completing all missions? Also, I completed all of them with 7-8 runes on (i.e. all greater than 6), this achievement also locked. I created custom map and this achievement doesn't work either. What's wrong? I want to get all achievements.

Did you clear the Abandoned Casino?

As for the custom map, maybe you need to download someone else's and beat it? That's how I got mine.

Hm, I don't see Abandoned Casino, lol. Is it hidden somewhere? I need to make some trick to open it? And how to download someone's map?

To download someone's map, go to Workshop and subscribe to a fan made map. The workshop is on the list above to the right of Discussions. Then go to the game itself, on the bottom left corner there is a compass sign. Click on that to show all the fan maps that you subscribed to.

To unlock the Abandoned Casino, you have to go to the Crumble Zone level. You have to get some enemies to fall into the pit with the red border in the top right. I forget exactly how many enemies though,
