Perfectionist Achievement didn't get...

I am wondering what could have cause me not getting this achievement. I played through the game myself, then followed a guide for this achievement. Went step. by. step. without missing anything, and never recieved the achievement.

1- Are you required to have 0 extra mouseclicks?
2- On the Arrow panels leading to the 'lights out' panel, do I have to click exact locations?
3- Can you miss hammer swings on the Bloody Baby game?
4- (I had a perfect game of Bomber)
5- Are you allowed to alt-tab?
6- Are you allowed to switch to full screen and use the game menu?

7* I deleted my second attempt due to setting the wrong pattern of lights on the 7x7 grid leading to the Satanic Ending, I assume you cannot screw this part up either?

Those are the only things I can think of that would have prevented me from having a perfect game...

I'm trying to figure out what I'm doing wrong as well. I would add :

8- Can we use reset button right after completing an ending or do we have to wait for the animation

I think your problems are 3 and 7.

As for the reset during the animation... I have no clue.

I finally did it. So here a few answers :

1 - no
2 - no
3 - yes
4 - you have to
5 - I don't see the point here....but I guess you can
6 - I didn''t actually did it, but I guess you can
7 - that's right, you can't miss a single one

8 - doesn't matter

So basically, do you have to be perfect with no miss click at all? No! Though, you can't click on anything useable more than needed. Any extra use of a button and you're screwed (except for the blood boss where you don't actually use a button at all)

Whait a second! So, you're saying that clicks on empty field is OK? Only button clicks are matter?

I just finished an absolutely perfect run according to everything you see online, still didn't get the achievement. Any ideas?

i think your allowed to click how many times you want but you cant click an unessasary button that does not contribute to the game direclty, and you dont have to do the ending in an specific order.

I followed the endings walkthrough here on Steam, and misclicked only once (on the last hidden arrow buttons) but still got it.
Just to be sure, do the midnight one first, then the city bomb, then the demon baby. Then follow ending 1, ending 2 etc. on the walkthrough to get all the rest.

Did you push any extra buttons on the "awaiting input" ending? That, extra bomb drop in the city, or an extra hammer swing on the demon baby would be the most likely culprits to me.

Pushing extra buttons on the "awaiting input" ending halts this achievment.

I don't know why but i can't get the perfectionist achievement. That's my third try.
I did any mistakes on my run. What I need to do more ?
No clicks on the board ?
All endings in a specific order ?

About that ach. How many endings are necessary? 1 to 15? Every ending(25)?

You can click on the board, miss hammer hits on blood boss spawns (not sure about hitting boss at wrong times) and pull the Restart lever as much as you want. You can't click any extra buttons that do not progress you, including missing bombs during bomber minigame.
