For me is the same. Playing 3 months ago a game who runs so good in ultra, now is a pain-in-the-ass texture broken, problems with my controller, and blocks in option screen.
What did u do?

Mhhh... they had a patch a short while ago that fixed it for me. Maybe there is a different problem in your case.

Guys i got mine working.

Tuns out the menus partially recognize the controller with few things but not with most- leding us to enter play with the controller and get stuck... you can 'click'/start with controller but the lvl selection screen you can't.

When i tried double clicking on the level... it started.

In-game the controller works fine.

Im not sure however about shooting behaviour- in-game we can shoot with both shoulder buttons and 'a' (on xbox controller), 3 alternatives for the same function.
During play keeping the shoot button pressed continuously shoots, but afterwhile (for me) it stops and don't recognize the button i was pressing- all it takes is then shoot with some of the others.
