If it's from dead nuggets, need more cemeteries. Make sure you have a radius covering every part of you city. Also, as soon as you see an infection notice, click the "infection zone", in the news feed, to zoom there, and either heal the dead nugget to clear the infection spot, or pick the body up, and toss it in a cemetery to keep more nuggets from walking through it (don't need to do both. Picking the body up is cheaper CP-wise, healing the infection zone is less effort ;) ). Though with good coverage, and upgraded cemeteries, you shouldn't see many infection zones.
cemeteries might be causing dead nugget infections, thank you for your answer!
I'll build more wells but I am fairly certain two of my cemeteries do not reach the inner core of my city causing lots of dead nuggets that I was ignoring. :p
Have 1 farm dedicated to herb (without this, the hospitals won't work properly)
Have enough cemeteries to cover your whole city (when you put one down, make sure EVERY area of your city in green)
