Softlock with the Guy at the Lighthouse

He gave me a quest to find a woman in the cave. After failing to do that, I saved the game, and killed him to see if he would drop the item I needed to progress... which he did. I then felt bad about it and reloaded the game. And that's it. He doesn't show up anymore, and neither does the item he was supposed to drop on death.

Was the save you loaded before or after beating the Mines? Because he won't show up until you beat the mines, due to an event that happens when you beat the mines.

I saved right before killing him. He was already present, and had already given me the quest. Also, I went to see him before beating the mines, and he was already there, with different dialogue... but this is not relevant.

For me, on release day he was there before beating the mines and him, his buddy, and the girl do not spawn along with any of the mantis monsters
