Report a translation problem
In DS4Windows, make sure you've enabled "Hide DS4 controller" in the settings menu - this enables "Exclusive Mode", indicated by a ✓ on the main screen of the app. From my understanding, this makes sure that it converts the DS4's inputs into a virtual 360 controller (some other applications can also interfere with DS4Windows activating exclusive mode).
You also might need to disable Steam's controller input mapping in settings (I can't remember where it is, but I think it can be changed per-game in properties).
These steps let DS4 work with 13 after I set them up, and it worked with 13-2 and LR just the same
Also one other thing I noticed with LR: if the controller disconnects mid-session, the game won't register inputs from the controller after reconnecting until after the game is rebooted (which can be annoying, but the game can be saved almost anywhere so it's not too bad).

As you suggested, I toggled the Hide DS4 controller" in the settings menu which did enact Exclusive Mode. However, now DS4Windows doesn't seem to recognize my controller. D'oh! It's also not being recognized in Steam Settings and, for the life of me, I can't figure out what would've changed. It connects via BT just fine and is detected when plugged in via USB, but the inputs no longer register.
On a whim, I also tried InputMapper as I'd read that allows a user to mimic other input devices, but no luck there.
Not quite sure what next steps are outside of digging up an old 360 controller and trying that. Maybe uninstalling both apps and then re-installing DS4Windows to see if I can get the detection again to try your solution. I'm just flabbergasted that people have to jump through so many hoops just get a controller working for a game that natively uses a controller.
Looking through the DS4windows settings, there's a "Use DInput only" setting in Profiles, which only works when the controller is wired, and doesn't work with Exclusive Mode. When I turned it on, I got the same issues you described (steam doesn't detect controller, doesn't work in game, etc)
Quick question: are you using the Jays2Kings release[github.com] (version 1.4.52), or the Ryochan7 release[github.com] (version 2.2.15)?
The Jays2Kings version is outdated/no longer supported (last updated Oct 2016), which might result in some issues, haha.
If you are using the old version, try installing the Ryochan7 release instead, and see if that works. (extra note: as of version 2.2.14 and later, the app will ask you to update to .NET framework version 5, if it isn't already installed on your system, as it will be used by future releases of DS4Windows - if you don't wanna do that just yet, install DS4Windows version 2.2.13)
If you are using the new one, make sure "Use DInput only" setting in Profiles -> Default -> Other (https://i.imgur.com/gc3eodi.png) is turned off.
