Also, not to be glib, but have you updated your graphics drivers?

(Alternatively, go to your Steam library, right click on the game, go to 'properties'. Click the 'local files' tab and click the option to 'browse local files')
Scroll down and there should be a file called Medieval_TW
This is a .exe file
Right click on it and click 'properties'. Go to the compatibility tab. Here you can run the game in compatibility modes. You might also want to try ticking the 'run in 640x480 screen resolution' box. Once you get into the game, you can then change the resolution back in the game's vdieo options (1024 x 768 is the best option for the campaign map).

I'm in over my head with this stuff though, I downloaded DirectX 9 because it said on the Total War page that it's required. I looked around online first and several places said it's true that some older games will not play on the newer version of DirectX. But I downloaded it and installed it, now what? Do I need to open it somewhere or how do I select to run the game in it rather than the version I already had? Anyway I tried opening the game again and it says the same thing. Also, on the article I read on how to install DirectX, it also had the suggestion to enable Direct Play in Legacy Components. Didn't work for me but here are the directions how to do that:
Press Windows key and R
Type in appwiz.cpl and press enter
Check if the Programs and features part of the Control Panel has opened
On the left, click on Turn Windows features on or off
Look for Legacy components, double click it or press the plus sign to see all components
Check the Direct Play checkbox and click Ok
Total War: Medieval 1 was released before Windows 10 were available and as such was never tested specifically these Windows OS Systems.
Unfortunately this means we cannot guarantee it will run without issues on Windows 10. However, there are steps you can take that may help to get the game running successfully.
1 - Please try to set the compatibility mode to an older windows os. Try using these steps:
- Right click on the game icon shortcut
- Click "Properties"
- Choose the "Compatibility" tab
- Check the box "Run in Compatibility mode for" then choose Windows 7 or 8.
- Click on "Apply", "OK" and then try to run the game using the same shortcut
2 - Refresh your Steam files. Please refer to the following article from Steam’s Knowledgebase and follow thoroughly all the steps mentioned:

3 - Verify the integrity of game cache as follows:
- Load Steam
- From the Steam Games Library, right-click on the game and select Properties from the menu.
- Select the Local files tab and click the Verify integrity of game cache... button.
- Steam will verify the game cache files (GCF's) - this process may take several minutes.
- Once the process is completed, the Check Window will automatically exit.
Unfortunately if this does not address your issue, this would mean that you might not be able to play the game in the best condition on your computer as the game is not fully compatible with your current Windows OS.
Lets face it. Its an old game with no multi anymore.
Its a convoluted way to get it to run. But run it will.
It starts normally. Unfortunately after about 5 minutes it locks up, and Windows shuts it down. Windows 10 has got to be the most unreliabl operating system I have ever seen on a computer, but if you buy a new computer you're stuck with it. All I can add is to wait till it has another big upgrade and hope they get the game working again.
I completely uninstalled the game, reinstalled it. and ran a verify files check on it. It didn't help. If anything it locked up more quickly than it did before. Window 10 IS a beta operating system. We the customers are doing all the trouble shooting on this new Microsoft release. It's a Hell of a way to run a railroad.
set as compatibility mode window xp service pack 2
run this program as an administrator
and then ensure the screen resolution for the game matches the screen resolution on your PC.
MEDIEVAL Total War in game Options > Video
ensure you select the strategy map and battle resolution resolution to match your PC default resolution mode
im running on 1280 x 1024 resolution
+ i have the exe set as short cut on desk top and can run with out having to log in to steam
so far game is working fine for me now it seems im runing win 7 but should work on win 10
