I may have a fix for you. I can't remember which it is exactly so please forgive me but you either turn on V sync and turn off tripple buffering. Or you turn on tripple buffering but turn off V sync.
I know its a common fix people suggest but one of them bugged my game out but the other actually fixed it.
MOTMOD actually makes the missions harder a fair bit so it's not quite like playing the first game.
It also makes some big changes which is why I only recommend it for a second playthrough. (Such as removing the STG44 from the only level where enemies use it for historical reasons but not adding it to any other levels.)
