I was pretty puzzled as well until I realized there's a hint in the game. The spines on the stack of books to the right of the safe say (forgive me, I'm paraphrasing) "The answer to that which is before you lies in that which is behind you." If you turn around you'll see three pictures on the wall. Cows, gun permit, ruined wedding portrait = milk, gun, sorrow.

can someone help with the weird car vortex tunnel thing in level two after the safe and the milk?

I'm not sure but I think the correct direction is the ones that play a sound effect.
