The roaches you get by having the roaches sneak into your pack while walking around then bringing them back home, you have to bring enough of them home to trigger the event, and also you have to be nice to them by petting them. They will eventually trigger an event in the bathroom when both of these conditions are met.
The perfect ritual does NOT require you to save your arm, OR save the baby rat. I killed the rat on my first playthrough and still got the perfect ritual. The only conditions are that you correctly deduce the visitor's depiction items, and that I believe all 4 of the prophets survival to perform the ritual. Oh and that you don't "accidentally" give the prohets a certain item instead of one of the depictions. (That leads to two other potential, a very BAD endings.)

> When you see roach in your inventory don't remove them immediately go back home soon they'll appear in your house start petting them in every room 3X a day, then start cooking food via oven and leave food on the table to rot eventually you'll have to sleep and the food will disappear after you awake the next morning. Repeat cycle till roaches start giving you notes
