Abandoned game? Can't play in 2025
Can't launch with basic included settings like DX12 without instant crash. Game crashes immediately after you get to Prague at exact same spot. Discussion is full of random points where your save/game is bricked due to crashes.

Any community patches to fix this or is DE:MD abandoned?

turn off mxaa and dx12,and limit the number of CPU cores to half or more

I did that - game crashes immediately after arriving into Prague and beginning your walk through the train station.
Yeah, it's abandoned. Try the Gog version.

how would that possibly fix the game it's the exact same files
Put this in the steam launch options box for the game assuming the default install locations: (the CPU affinity mask here tells the game to only use half of my available CPU threads; the "-benchmark" option at the end skips the unskippable startup spam movies)

Limiting the CPU threads to 8 (cpu0-cpu7) of my available 16 while using dx11 with max settings and no MSAA has been rock solid for myself. For AA I just enabled FXAA in the nvidia control panel. If you're on nvidia hardware then DLDSR is also an option for AA.
If you're on a high refresh rate monitor I found it best to limit the framerate to 90fps with afterburner to avoid the 120fps and above bugs with the game engine and use lossless scaling to double the framerate to 180fps with LSFG 3.0 frame-gen.

In my experience, every time I start a new game and get to that same intro scene in Prague it crashes to desktop, but if I start the game again it gets past it no problem.

Worked for me. This thing ran like garbage for me in the Steam version. Gog version runs great. Could be Denuvo.

are u on windows or Linux? in Linux for me the exact same issue is solved by running the game with proton 9

I just made a 100% completion in the past month. It is buggy, sometimes crashes, but otherwise it works. 80 hours plus of gameplay. BUT I played this on linux under proton.

1.45% of Steam users are on linux bro, I know Linux is superior but I'm not doing all that to play a game. The fact it can't run on Windows is clearly an issue. I've tried everything in this post so far and it still doesn't work. I limited cores and it now doesn't crash but it freezes for like 15 seconds which bugs out the entire physics and scripts of the game to where I have to close it.

You asked if the game was abandoned. Those aren't people trying to convert you to a linux user, just anecdotes that the game only functions from either GoG or on Linux under proton right now. I think the answer is clear. This is a bad release of the game, you should refund it while you still can.

Outside of a few instances of an infinite loading screen fixed by closing the game with Task Manager and reopening, I've been able to play.
I'm playing it perfectly fine. I've had... Two crashes I think in a good dozen hours of gameplay, and the autosave is very forgiving. No special tweaks, just downloaded, hit play, works fine.
