Though I usually had to go in invisible against ppl facing each other since they would spot me before the multi takedown was possible (only really relevant if you want the ghost bonus though).
But yeah, as soon as some sort of level geometry is involved things failed for me as well.
Likewise it's not possible to use a double takedown against two sitting enemies (Dvali's sewer base has some, also spotted two in Golem City)
This was also the case in HR.
Otherwise, you should just be really close to perform a takedown. The developers also tried to minimize the easy to spot pair of enemies facing the same direction. I have only discovered one pair, in Golem City, on the balcony.
P.S The takedown system is strange, sometimes you can perform a knock down from 2 meters and sometimes the button prompt never appears, until the enemy changes position. I suspect it depends on a "scene" where you knock out/kill an enemy.
The engine is far from perfect, the blink ability is incredibly flawed as well.

I thought it was some deal with the game handling civvies and hostile targets differently, and not able to "combine" them in a Multiple Takedown usage, but it just seems the game is a buggy mess?
After all these years...
