I'm pretty sure it must be a bug. The only time you remotely encounter her is during a side mission after you get a strange signal from a malfunctioning movie/map poster in one of the metro's. Even then, you only hear from her through your infolink and TV, never in "person". I don't even know if she even has a body at this point in the story.
Well, for animating purposes they would have to have her body as an asset, since during the side mission, when Eliza talks to you it is animated during gameplay, not a pre-rendered video
Oddly, they left her as malfunctioning npc. Im not the first to encounter her, either - found a youtube vid and one of the comments mentioned this, too.
I walked over and interacted with it, and was thrown into the conversation that you do while you're in your apartment and she's trying to put herself together. After I went through the whole thing (again) and she said that the thugs were coming to get me in my apartment, *POOF* I was in my apartment, and the thugs were coming in the door, as advertised. So aside from what appears to be a spectacular clipping incident, there was also the manifest Eliza/Helle, and I was moved from one part of the story into another that I'd done hours earlier. I restored from a save and did it again. I still had the alarmed thing under the floor, but I was through and out before it came to the surface again. There's a serious bug in there.
I'm guessing that the area that is your apartment with Eliza/Helle and the thugs is logically right 'under' that area, and from the 'node' where those NPCs are they can 'see' you through the 'floor' - that being the clipping accident. The rest is the engine taking a giant dump because it has to try to reconcile two logically separate parts of the story arc that are apparently happening at the same time in the same place - co-existing. Kind of meta for the Eliza/Helle character, but just weird for the player.
But this begs the question. Why is there a 3-D model of Eliza in the game if she never appears as a 3-D manifestation? Some part of the story that got cut and bits were left behind? Something I've missed? Something that happens on one of the extras you unlock by scanning triangle codes? WHY DO THEY ASSUME EVERYBODY HAS A CELL PHONE? (I can't scan the triangle codes, you see - no cell phone. About 20% of the population actually *needs* a cell phone. For everybody else they're just a useless waste that you are instructed to have, so you have it. Speaking of meta...)
There's an explanation for her being in game files - when she talks to Adam in his apartment, it's not a pre-rendered cutscene - as with most coversations, it is done real-time in the game, which means Adam is having a conversation with a filmed npc. A la Dr. Breen in beginning of HL2 - you can actuall find him in his "tv studio".
Her being in game outside of the tv scene must be a bug.
Also, that issue with Adam being teleported to another area for a conversation happens when you talk to Delara in Miller's office - she'll talk to you first, and if you try to activate her after the conversation again, she's suddenly sitting in her office.
