What does Painless Mode do?
I was playing and got the item from Terry that turns on painless mode, but I didn't really see a place where it says what it does specifically, like stat wise and enemy wise. Is it like the joke easy mode in Joyful or something actually serious?

From what I can tell I know I've had fights where the mutants "bit down" on my guys and didn't instakill em. So perhaps the mode keeps the normal difficulty but keeps all instakill attacks from actually instakilling, just damaging

STRIKE THAT, the mutant at the bathhouse Bit down on Buckets and instakilled him. So yeah I don't know what it does...maybe its a placibo for all I know

I'm wondering that myself. I turned it on on Area 3 to make it faster but I didn't notice any particular change. Maybe the enemies have less HP? That's the only thing I could believe. Either that or it may be bugged

It probably is a bug but a little part of me hopes it isn't a bug and it actually just does nothing, feels like the type of japery Austin would pull on you in his games. Terry would 100% use sugar pills and think they are steroids.

I could also believe that but I don't think that would be a great idea, I think it was even in the trailers so it feels like false advertisement.

Yeah it's most definitely unity and day one shenanigans causing them to not properly work.
