hypothesis: Buddy is Pregnant in Lisa the Joyful
Lisa the Joyful is a DLC expansion of the rpg Lisa the Painful. It has mature and dark subject matter unsuitable for kids, so be warned if you read further. If you have an issue with severe abuse and/or mutilation in a video game, this is not for you.

This is a hard one due to the subject matter and everything about the game, I could write an essay on how it throws everything about "Gamergate" on it's head, but that's just the smell of the amusingly filthy apocalypse of the game's environment. It is, after all, "A game about survival, sacrifice, and perverts..." by description.

Theory

This theory comes directly from the dialogue in the early first half of Joyful when Buddy and Rando are jumped at locked in rooms.

"I don't think we should tell him"

"No. . . We have to"

"It would crush him. It's better that he doesn't know."

"It doesn't matter. He needs to know"

"He is going to be furious"

"This isn't a game. We are trying to save the world and keeping secrets isn't going to help anything. Whether it hurts him or not, he has the right to know. Now wake him up."

"Alright fine"

After he wakes up, they tell Rando they knocked her out and tied her up.

Men: "Rando, she was struggling so much. She was so violent!"

Rando: "What kind of message does that send? She is not a prisoner. Sh-She is our future, not a slave."

Men: ". . . That's not all. Dale is dead. We killed him"

Rando: "What?"

Men: "While she was tied up. . . He. . . He. . .Well obviously it wasn't anything serious, but he--"

We don't hear the end of this conversation as Rando loses consciousness trying to leave while the men are blocking the door.

He regains consciousness and finds his way to Buddy,

Rando: "That's all?"

Buddy: "I told you, this is all people want to do to me. I hate it. I refuse to be locked up like this".

She physically distances herself from Rando.

Buddy: "Did you know these men? Your friends you talked about?"

Rando specifically took her to them when they were jumped. We just saw them talking to Rando as a higher rank.

Rando: " . . . No. I don't know these people. Lets not worry about my friends anymore. W-We just need to worry about you n-now"

Endings:
There is an ending with Buddy replacing Sweetheart as the favorite Joy Mutant of Dr. Yado. Either the vaccine turns people into mutants or she was never given the vaccine. Either way, being pregnant may have been the reason she didn't turn into a mutant when Yado expected her to. The other ending when given the choice is to "leave them" and Buddy now has a child and has not transformed into a mutant. She actually takes Dr. Yado's place in controlling the mutants. All the known men are dead. You were given a choice between being ♥♥♥♥♥ by Bolo to save Rando or just letting Rando go to kill Bolo, so the child is likely not his. It was most likely Dale's.

Also, this lullaby is first heard in the game when rando finds Buddy in the room with two men killed. Rando wants to know if she is okay and she says "They tied me up and locked me in this room."

While her age is kept vauge, it appears some time may have passed between Painful and Joyful based on her apperance between the two games. however, This is contradicted by the fact that Rando is on the field "done" at the begining of Joyful. Is Joyful taking place immediately after Painful's ending, or how long?

The game is designed to let the player make his or her own mind in the illusion of choice.

joyful starts literally the instant painful ends

did you not pay attention at the start of the game

My original theory was dale, but Austin said no to that. Plus you can see Dale's dead body (they said he was thrown off the cliff) wearing the pink shawl after that scene and his skin would hint at a darker skinned baby. So after taking that into consideration and looking at the sprite. I think it's Dusty's kid (similar features to Dusty's young face). That'd also explain why she doesn't keep the baby if she learns that Dusty was behind the kidnapping part in the start of joyful.

What makes you think she didn't keep the baby? No one else will take care of it. She is letting her mutant dad watch the baby while she also keeps an eye out with her trumpet of tyrany/rule.

I was referring to the "hold on to the rope/Join them" choices that lead to the ending where no baby is present. If holding onto rando or not decides whether or not a baby is present in the join them ending, then I'd think the baby living through the mutant's blind rage would be related to her feelings about either bolo,dale, or rando. since the former two were stated by Austin to not be the father, and since the joy mutant is hovering over Rando's body maybe he's the dad.

That's just about all the evidence I can find that points to that conclusion but maybe.
