There is 2 type of shaft on the surface of Golgotha.
Regular slimy shafts (green one in the open) that bring you one floor down in relative safety (you still have to survive the dungeons xd ).
The other type is the express way that bring you "straight" down to the last level. In doing so you fall like 4 level and take heavy damages. Only ways to go down safely is to either have lots of hp (like >150hp) and use a rubbergum tonic (reduce fall dmg) or to be able to fly and just fly down "manually".
On a side note, the deep shaft isn't readily accessible (in fact, it's suposed to be the way out) as it's in a room with strong wall and a locked door. Also, there is a prompt warning player it's a really deep hole to make really sure you don't fall into it by accident.
If you however died using the regular slimy shaft you need to check your journal to see what killed you. If it's the fall it's clearly a bug and need to be reported, If it's anything else well, welcome to Qud.
I think I got in easily because I had psychometry and so I could just walk through locked doors.
Didn't tought about psychometry but yeah this explain that.
At least you got an achievement out of it xd
