I think it means he will have to compensate for the price by giving drams of fresh water.

Ex: You give him an item worth 10 drams but you ask for an item worth 5 drams
So the merchant compensates the value by adding 5 drams of fresh water.

It means he needs to give you fresh water to compensate for the difference between your sold items and the bought items.

But the game will tell you if that happens, and ask you if you really want to complete the transaction.
