You do kinda end up (always been?) as royalty by the end of the main quest. Also, from what I gather from the lore, Sultan of Qud is kinda like king of earth / solar system back in the time of the eater.
Also, sultans resided after the time of the Eaters. Immediately after their fall if I remember right. Sultans are more or less rulers of a post-Eater society that's going on a downward spiral due to the incremental Girsh curses that kept wracking the land of Qud.
