Multi-armed Gunslinger?

Does anyone know if it is possible to make a mutant with four arms, wielding four pistols, and actually make attacks with the extra two slots? Also, could you make four attacks with the Akimbo ability?

I havne't personally tried, but the description for Multiple Arms specifies that it gives you the chance to make extra "melee" attacks, so I imagine no.

Multiple arms grants no missile weapon slots.

other people already answered you, but if you theoretically had additional missile weapon slots then yes Akimbo fires all pistols you have equipped
the Akimbo skill seems to just be a kind of toggle on whether you shoot all your pistols lol
