Historical Sites, do they always show?

I've found three historical sites and plundered two, but the third spawned in mountains (Can't get to it, oh well) and I have another quest to find a Sultan's awesome sword of mysterious stranger's woe but it never provided the location of where it was lost. The name of the place is off the screen in my quest list so I can't read it either, definitely cannot find it on the map.

So my question is when you get a quest after reading some histories/statues to acquire an item at a location do you always get told the location of the historical site?

To get at that mountain site, go through the underground.

I was also told I could tunnel through the overland, might try underground later. Even if I get in there and torch the place I'll still have that sword quest with a location I can't read.

The artifact locations refer to specific floors, not map locations.
