How it work?
Will your character wake up when he got hit?

Yes, if you take damage while asleep, you wake up, but with a temporary debuff ("dazed" I think).

In my experience, it can make running from enemies difficult sometimes, but I don't think I've ever had it lead to a character death (just some dismemberment from not being able to run from a madpole).

there is an exploit if you take it with sleep gas generation which probably should be fixed

Yes. Sleep Gas generation makes you immune to being slept, so taking it with Narcolepsy nets you two free points (plus the free ability to generate sleeping gas and immunity to the same, which is nice in Bethesda Susa.)

This is no longer the case. Just tried BEQMMMEMBABNBUDNDZED from this thread https://forums.somethingawful.com/showthread.php?threadid=3739217&pagenumber=1 fell asleep in the middle of my first fight and did not survive.

I've never dared take it. Sleep effects are actually pretty nasty - and dazed itself is one step away from stunned. Against the wrong foe it could easily result in unavoidable death.

Yes, the sleep gas + narcolepsy trick was fixed long ago.

I almost always take narcolepsy as a defect. It has not caused any serious problems.
