In most VN's or the like, usually its two. But there's no real way to know in some cases because the girls hair is usually long to cover their head, so we can't see if there are human ears.
In some other artworks though, like that one of Tsubaki (I believe that was her name, she's from Pure Pure) in the post above mine, their ears come from where human ears would be. So, its really dependent on artist and other factors..
So if your catgirl has 4 ears, she's really just a human wearing cat ears. Their tails are most likely fake also if they even have a tail.

Both are canonically catgirls (well, youkais). IMHO, I like both types :)
