With Nekopara, there is a Steam version, and a full version (sold outside Steam) with all the content in it. The 18+ DLC effectively turns the Steam version into the full version. It is not a separately playable thing; it integrates content into the story. And the same thing goes for most visual novels and games with 18+ patches/DLC.
There is jut no way of being able to tell if you're playing the dlc or not, with ark dlc its easy to tell because u know which game ur booting up. And i remeber when i launch a dlc i get different main menu, so it probably indeed can be launched by the client. Or anyway, even if u dont do that, when u launch the main game, u can select which one u wanna play. But here there is no way of telling what your're playing. I even asked a friend for help and he had no idea.
It is, however, possible to tell without starting it, in this case. The full version, or the Steam version + DLC includes a file called "adultsonly.xp3" (and its .sig file). That's literally all that is changed; on start, the game tries to load that file, and if it succeeds, you get the full content, while if it can't find it, you get the censored version.
As far as switching easily, it seems it did not occur to NEKO WORKs that anyone would want to do that, so they didn't implement any sort of in-game switch for it. What you can do is remove the aforementioned file (this works even for the non-Steam version), either by renaming it to something without the ".xp3" extension or moving it to some other folder, or the reverse. Or, in the Steam client, you can switch the DLC on and off in the properties (right-click on the game in the Library, select Properties…, then go to the DLC tab). You'll need to restart the game if it's currently running.
EDIT: Also, the same applies to Vol. 2 and Vol. 4. Not completely to Vol. 3, as they tried out a different engine, which has a different file structure. It's harder to look at the files and tell whether the DLC is present. At least I've never figured it out. However, the sizes must be different, at least. Switching the DLC from the Steam client still works, of course. Vol. 0 and Extra have no adult content to begin with, so they are irrelevant to this.
