Well... The attacker gets killed anyway. The skinny little coal boy somehow manages to kill the fully armored, muscular attacker while almost drowning in that pool.

You can trade in the coal boy either way I believe. The only thing I guess that'd change is whether you are guilty or the coal boy is. In one Mira kills the guard, in the other Tom apparently does. I think either way, she's on the headsman's block unless she agrees to marry Morgryn (you're an idiot if you don't).

I didn't have Mira kill the guard, I didn't want blood on my hands. Tom gets a bit pissy with you when you next see him but other than that everythings the same. I married Morgryn, I can't having Mira dying on me, she's too amazing a character.

What about when Tyrion gets arrested, does Tom help unloocking the door?

Pretty much nothing changes if you kill Mira's attacker or leave attacker and Tom behind. Tom appears to you next episode. Mira is surprised to see him alive, but Tom is pissed at her for leaving him to die. There's only change of dialogue and Tom tells Mira that he "took care of it". Everything else will remain the same. He will help you unlocking the door and Mira still gets accused for murdering the guard in the last episode.
