List of Souvenirs ?

Is there a List maybe? i'm missing 1 and I cant seem to find it i have run every Dungeon multipe times except the last one because i crash at the Boss.

The last one i needed was a mask, the one on the right side (should one on each side) of the chapter select. I got it in area 3 in the second portal (air god one).

I think that is the wood carving, I got the in portal 2 around the time i was helping the stargazer if i remember correctly
