This, it's probably either a goblin artisan or a goblin warrior with the Insane trait to coincide with either higher Forge skill or higher Damage. Lock these problem children in a room of their own and do not let them out until they are absolutely needed.

I can't find any tags on my minions for insane either. I stopped recruiting them but then i rescued some dark elves from a dungeon and they to met the same fate . . .

Check out what they're eating. And use. Maybe mushrooms or scrolls that make them temporarily hostile.
