Korama from Sonic Retro wrote a program for decoding and re-encoding the SDAT/save data file in your Lost World index folder. If you drag and drop your save file into this program, you can then open it in Notepad/Wordpad/whatever text editor you prefer. CTRL+F for 'item', and you'll see five numbers listed out just underneath. The values for items are;
39-42 = Speed Shoes
43-47 = Shield
47-50 = Red Ring Radar
51-54 = 1-Up
55-58 = Wisp Bomb
59-62 = Magnet Shield
63-66 = Invincibility
67-70 = Wing Warp
The higher the number in a group, the more stars it has. For example; 39 is a Speed Shoes item with 1-Star, while 42 is the 4-Star version. Put in all the items you want, then save and drop your SDAT file back into that program. I'd advise backing up your SDAT file beforehand in case your text editor destroys it somehow, but Wordpad's been fine with editing it and my personal recommendation.
To answer your last question; 4-Stars means the Wing Warp sends you to four checkpoints away (skipping nearly the whole of some levels), and... the Red Ring Radar picks the Red Rings up from further away and beeps louder when you're near one, from memory? Anyone's free to correct me here.
