How do I unlock the ex color costumes?
Can they be all unlocked at once?

No, they can't.

For Dante and Nero, load up Mission 20 as Nero and get 30 kills during the end credits and don't let Kyrie take a hit.

For Lady/Trish do the same at the end of the game, get 30 kills and watch the credits.

For Vergil, kill 30 enemies and watch the credits. At the end of each you'll unlock the costumes for the respective campaigns.

@Tensa: I read that if you got an A rank or better you play as Lady in the credits, if you got a B rank or lower you get to play as Trish in the credits.
