Hell and Hell sounds the hardest since you die in one hit, and you only get 3 chances before getting a game over, but enemies are still manageable if you've learnded them properly and if you already beat the game on the previous difficuties, it's more like doing what you've already don't but with less mistakes.
Dante Must Die is always the hardest difficulty in the seires.
Not only do all the enemies and boss have vastly more health and deal more damage, they are all able to Devil Trigger as well, which makes them much harder to kill.
If you can beat DMD mode, then Hell and Hell and LDK will be easy in comparison.
