Or a mixture of both, like extra content, but base gameplay of original NG2?
Because adding gore and enemy placement won't change the game if they don't change how the Bow Behaves as well as some other minor things like puzzles.

Also, since it's based on the horrible Unreal Engine, does the game micro-stutter, or does it have shader-precompilation?

It is 100% Sigma 2 with some minor tweaks/adjustments (Sigma 2 engine running in the background). Check youtube for a detailed comparison.
Updated sigma with some minor tweaks to make it feel a little bit more like the original.

Hah that Dude is completely clowning himself. Could not be more obvious that he has absolutely no idea WTF he's talking about. He's just parroting arguments he sees online without having even touched the original NG2. It's obvious to anyone who has actually played both.
