Side note - I did a play through on my Macbook Pro and didn't get the semi achievement...it seemed like I should have.

The semi-achievement is for beating the game when using continues. If you beat it without continuing, then you won't see it.

Ok. Is a life a continue? I beat the game dying maybe 9 times along the way (mostly on the rocks).

Nope… you can lose a life and it will be fine. A continue is when you lose all of your lives and you get to that dark purple screen where you have to go left (start over the entire game) or right (start over on your current level)
