But yes, I how the game's randomized sometimes frustrates me as well. I had time when I can't find the next part of the quest in multiple runs because the randomized rooms just don't work in my favor.
On your problem, same thing happened with me as well. I accidentally killed slash. If you didn't kill him, if you die in that run, you'll find that room where slash is trapped in your future runs. But since you killed him, you'll have to fight him again once in a run before the trapped slash appear again /in a random room somewhere/
here's character unlock guide that I got when I was wondering about that myself.
The fact that the ! mark is not present in Slash's room after it's been hurt is a bug, actually. I'll see what we can do about that, because it might not be easy to fix.

I didn't kill Slash or anything of the sort (did previously, but got past it, as above). Freed him from the laser, then had the talk before the boss.
Far as I can tell, the game is bugged and the quest won't resume :(
From my Unlocks.brw file... I imagine changing some of these values could get me back on track?

EDIT: well, somewhat intuitively I changed some 1s to 0s, and it reverted to the "free him from the cage" quest. Beat that, saw him in the hallway, beat boss, saw him at end of boss... then Slash is gone again.
