It isn't a fan game. It is an official product known as a fan disk. It is something made for a main VN to serve as a sequel and only made if there is enough demand for it. This particular VN has at least 2 fan disks and this is the first of them. The other one I believe was just another sequel for Kotori, but this is the only one announced so far.

My understanding is basically, Kotori got an after route; Amane got a before route; Ageha and the twin got after stories (and no separate Asa or Yoru route; yes, the harem is still here for the twin); Hotaru and Kanako have grand new adventures.

The Kotori route is a continuation of the Main Story.

The Hotaru route and the Kanako route seem to be routes after the bad ending of the Main Story (assuming that you didn't date anyone in the Main Story).

Isuka-Amane route appears to be before the events of the main game, but I have never played that though.

The twin route and the Ageha-hotaru routes follow their routes in the main game, so yes, they are after the evens of the Main Story. The Ageha-hotaru story is different from the Hotaru route in the Flight Diary. In the Hotaru route in the Flight Diary you will date Hotaru, and most likely you have never dated Ageha or other girls before.
