Play the whole campaign and don't free roam.
Play over and over till you get sick of it.
Use melee most of the time.
Put on Karma bonus gems and get the Karma bonus skill, of course.
Note that max level is 96 and 3/4, so once you hit level 96 go back to your life, call your mom, and hit the gym.
Anyway, you sure about it? Because when you select the difficulty it says Insane 1 will grant 150% exp and Insane 2 250%, which is quite a lot more.
I was more interested in finding a good spot for farming exp tho!
I went up in difficulty each time I met the level requirement, and I ended up leveling faster than I could remember to put points into skills since I was still used to the slower Karma gain before it was based on difficulty.
That said, it's still a case of finding a good balance as fights get harder and potentially a lot slower with the resists/immunities later on. If you find yourself dying a few times in an area, you'll gain more overall from playing a bit slower or finding somewhere else.
Keep an eye on the S shaped bar where it shows your weapons - If it's nearly full, make sure to play it safe until you level.
