Kill enemies...duh
Play the whole campaign and don't free roam.
Play over and over till you get sick of it.
Use melee most of the time.

Best way to farm XP is normal play on Insane 1. You get the same amount as Insane 2-4, so don't bother going higher if that's all you want to do.

Put on Karma bonus gems and get the Karma bonus skill, of course.

Note that max level is 96 and 3/4, so once you hit level 96 go back to your life, call your mom, and hit the gym.

Lol, I won't hit that anytime soon.

Anyway, you sure about it? Because when you select the difficulty it says Insane 1 will grant 150% exp and Insane 2 250%, which is quite a lot more.

I was more interested in finding a good spot for farming exp tho!

It's more about kill speed than particular spots. For me, I'll switch free-roam when it'll take longer to back-track and find more enemies than it would to load a new level, but that's about it.

I went up in difficulty each time I met the level requirement, and I ended up leveling faster than I could remember to put points into skills since I was still used to the slower Karma gain before it was based on difficulty.

That said, it's still a case of finding a good balance as fights get harder and potentially a lot slower with the resists/immunities later on. If you find yourself dying a few times in an area, you'll gain more overall from playing a bit slower or finding somewhere else.
Keep an eye on the S shaped bar where it shows your weapons - If it's nearly full, make sure to play it safe until you level.
