Let us say you are at LSIA, the Bunker Supply truck spawn, you are standing anywhere between Big Barn and Belegiro (whatever) warehouse and you will pop up some stars if the Bunker Supply struck (military) bumps any vehicle. The soldiers may even get out and shoot you, you re-spawn and they are gone.
Now, lets say you have 1 star, you drive past the giant golden V on your map and you have 3 stars if the agents there saw you.
Now, maybe you did a Nightclub delivery 15 minutes ago, you dropped mines during the mission, this is 15 minutes later, that mission is gone and done, you get back from Sandy with 2 stars but within 400 meters of those mines, those mines are suddenly active again, boom, now you just killed cops that were spawned just to come see your 2 stars. Not enough time passed after your stars disappeared but wait, now it did, 2 stars.
So, did you lay mines anytime during that session? an hour earlier and had not been back in that area?
The star removal had not saved to R*, until it did that new reason for 2 stars looked like 4, but I was not there, I did not see it, but I have had the situation above happen before. I have even had stars appear while inside my apartment or whatever business it was because of the mines I had set before entering.
