Although I haven´t played all scenarios on extreme yet, it´s got to be either Refugees or Winterhome there.
Not only do you start with a serious case of clusterf#uck, where you have to clear up the ruins, while rebuilding, lowering a record high discontent and raising the almost non existent hope with a large portion your citizens being sick, injured, amputees or dying.
Then you need to provide enough resources for your citizen while you do the same for the dreadnaught, making tough choices on who gets to go as you attempt saving as many people as possible, with consistent generator problems.
Technically the endless modes have scripted things, like weather and temperature patterns, including the duration. In addition, the builders mode has a set of different malfunction patterns. A whole list of events, quests and hazards that can occur when triggered or between storms. The unpredictability of it is that it's randomly generated which definitely makes it difficult to prepare for unlike the scenarios, that are the same every single time. But play often enough, you can get familiarized with the events you've encountered before, which helps deal with encountered again.
The biggest difference between scenarios and endless however is that with scenarios you're on a timer with stricter boundaries. Can't finish preparations in time? Tough luck. Your best just isn't good enough. Game over.
With endless, if a disaster strikes, it's possible to bounce back every time as long as there are people still alive. And you know... haven't dethroned you for poor performance.
I suceeded The arks/New home/Last Autumn/On the edge on my first try (after doing them in normal and hard first)
Endurance early game is a little hard as you have to rush some tech and micromanage workers.
Builders makes it a little easier because temperature are nicer and you have less techs to worry about at first.
On the Refugees, it's hard to have enough food/ health care at first.
Winterhome is by far the hardest scenario, I had to follow a step by step of the first week to survive after failing like 20 times. I didn't even try to get a full dreadnought on extreme, I did the bare minimum. Even on Hard difficulty I can't make a full dreadnought, I usually have 75% done at best.
