This scenario is really busting my butt and I can't get past the first few days. I can't get the trust up past about 5-10% and everybody works at the speed of poured molasses. So tell me, these people are all a$$holes so why do I care about saving them again? They elected me captain so they should be busting their chops trying to earn my trust instead of the other way around for the first 5-10 days at least!
You also need to take overcrowding pretty early on to manage the massive number of sick people. Once you start getting on top of that, keeping everyone warm, and clearing out ruins it gets a bit more manageable.
Get overcrowding for your medical posts
Use gathering posts to gather ressources faster (if you delete some useless roads, there are some really good spots for them).
Try to regroup tents in the same area to heat only what's necessary.
Also the work efficiency is probably bad because you have lots of sick at the start.
