Controller Not Working

I played the game for about 2 hours earlier with no problems whatsoever. My controller worked perfectly fine. Now I load it up again and my controller doesn't work with the game anymore for whatever reason. Anybody else run into this problem?

Frist of all, make sure your key bindings didn't just reset (for whatever reason).

If the game doesn't recognize your controller at all, here's what helped me (from Dark Souls forums):

"Open up Device Manager and under Human Interface Devices, right click and disable all HID-compliant game controllers, and if you have a G13 disable the Logitch G13 Joystick. Restart the game and it should work.
You may also need to disable any virtual HID-compliant game controllers as well."

Please note: this game requires the controller to be turned on before the game is started up. If you turn it on afterwards, then it will be unable to use the controller.
