How do I revive as a ghost?
I wish there was the option to just respawn as a new character. :(

Revive with a touch stone, life giving amulet, telltale heart or a Jury rigged portal.

Does someone have to craft a touch stone? or can I just find it as a ghost?

Touch Stones cannot be crafted. You have to find them in the world.

What game mode are you playing in? I think the portal only revives in some of the modes... my friends and I play endless (I think), not on PVP, and we can use the portal fine, but I don't think it works for all.
