Somebody Joined My PVP Server! My Server Was Set To Friends Only! Then This HACKER Joined And BROKEN EVERYTHING!

If it was in fact set to friends only.. obviously it was someone from your friends list.
You could have simply rolled back the server to undo the damage.
Next time you should also set a password to your server to prevent random "friends" from joining.

There are. I've also seen some that do teleport and spawn items in servers that they are not host of it.

I thought that you can join friends-only servers throught steam profile without being a friend(just press the button to join), I doubt steam fixed that bug. (it's steamside on all steam games)
