Indeed he is alcohol. After Jacob lost his son in fire, he started drinking. Bartender is basically Jacob's drinking and the symptoms caused from it. Bartender is a complex character, he is not malicious but he has or at least had all the power over Jacob.
Remember how he said that they did not meet in the day of fire but the day of ashes. This is the day after the fire, the day Jacob started drinking. There is also a hint in chapter 2 where Bartender explains how he has been waiting for Jacob. At some point however Jacob started treatment to get a rid of his addiction. He did succeed, but later he fell again.
There are more, but hopefully this cleared some of your thoughts! Oh and thanks for playing! :)
A few questions still linger however:
So, is the ending suggesting that Gerald drove into the main character and therefore is a murderer? According to the Gerald's wife he was abusive already before, right?
I don't have a good enough recollection of the letters you sometimes find hidden. Do they in retrospect tell the story of Gerald's wife? What about the locked drawers in the clinic? Is there a new game plus? Can the letter the wife is talking about be found?
Indeed, it's pretty deep in the story. Basically the whole journey is mixed memories of Jacob and Gerald. Gerald has a problem with gambling he could not control. He was ashamed and took a miserable decision to kill his wife and child. He was on his way to get rid of the bodies when Jacob, with the aid of a whiskey, hit their car. Gerald did not die, but fell into a coma just like Jacob and so their nightmare started.
Unfortunately no new game option, there are hints and even more symbolism used to tell their stories. There are takes from the letter here and there. The only time you see the letter as a whole is in Chapter 2. Remember the letter that you could not read? It was all blurred and confusing because you have not learnt the truth. She was there, trying to help you, trying to tell you.
I've been thinking for some time, that I should write "Silence of the Sleep explained". I think people would truly enjoy that, and could appreciate some of the details that are easy to miss! :)

It was an enjoyable experience and I personally found the level of difficulty, length, storytelling, visuals and switch between how the player interacts / levels great, the sound design phenomenal; only movement was sometimes a hassle when searching for hotspots.
Will be looking forward to whatever you will come up next.
Just finished the game, really enjoyed it, and I did not think the monster sections were as bad as many people did (just had to be patient, look at the maps... died many times, but, hey, monsters are supposed to be deadly!). Funny enough, it was the carrot scene that really required me to look up things online (I thought there was a bug, but I misunderstood the mechanics of that scene).
Anyway, did you ever write the "Silence of the Sleep explained"? I liked the story as it is, but I would love to see what other details I missed.
