As long as you can still reach the village of Kuldahar, you can go there, visit Hjollder in a house and talk to him to start the expansion.

Last chance to return to Kuldahar is at Lower Dorn's Deep in the "Fallen Temple" area. It is the point of no return, since if you proceed there, the endgame phase starts.

Hjolder's house is at the end of the tree root that separates the Inn from the Temple of Ilmater.

No, no, it is okay for you to do all of the forge, "except" putting the badges you have been acquiring in the slots of the shrine of the Fallen Temple and ascend the stairs the final sequence starts. You can return to Kuldahar anytime before going up those stairs. You can put the badges in the slots (lots of XP for that) just do not go up the staircase.

This is where I usually make the change to Heart of Winter. The transition is smooth and you can return after finish HOW and return to the forge and finish the main game.

At Lower Dorn's Deep, the Fallen Temple is AR8012. You can use the badges there to activate the stairs. You can even go up the stairs to enter AR8013, BUT the fight against Revered Brother Poquelin involves the sudden teleportation to Easthaven (AR1100 and without spoilering anything here).

Level 9+ is the minimum requirement by Hjollder to start Heart of Winter earlier. HoW can be completed before completing the base game. And you can travel forth and back quite freely, but depending on Hjollder being available in the HoW areas. On the contrary, Trials of the Luremaster, which is integrated into HoW, is more of a lock-in expansion you must complete before you can return to HoW and/or the base game.

Yes. I've never lost items by starting HoW from Hjolder's house in Kuldahar.
