Last chance to return to Kuldahar is at Lower Dorn's Deep in the "Fallen Temple" area. It is the point of no return, since if you proceed there, the endgame phase starts.
This is where I usually make the change to Heart of Winter. The transition is smooth and you can return after finish HOW and return to the forge and finish the main game.
Level 9+ is the minimum requirement by Hjollder to start Heart of Winter earlier. HoW can be completed before completing the base game. And you can travel forth and back quite freely, but depending on Hjollder being available in the HoW areas. On the contrary, Trials of the Luremaster, which is integrated into HoW, is more of a lock-in expansion you must complete before you can return to HoW and/or the base game.
