There are a lot of countries with their speciffic weaponry, planes, ships, missiles...worth playing
The full CMANO experience allows one to control any nation's armed forces. The Northern Inferno campaign is limited to playing as NATO in 1975.
Depending on the design and designer, each scenario may only be playable from one side, or both sides, sometimes there are more than one playable 'sides' that work together.
Bottom line, yes you can play as Iran or Russia, as well as basically any country you can think of (and to a certain extent, any fictional realistic country you can come up with)
1) On in-game mechanics/DB bias: We have fanboys from all points of the compass regularly accusing us of favoring everyone but their side. So we know we're doing it right.
2) On scenario-level bias: Have a look at the Silent Service DLC, in which many of the scenarios place you in control of Soviet/Russian forces. Same for some of the standalone Command scenarios.
"Iron Hand": You are commanding modern RUSSIAN air forces against Azeri elements.
"The Four Horsemen": You take control of four top-line SOVIET nuclear submarines against a massive NATO ASW group.
"Duelists": Take control of either the UK or SOVIET side in a battle for control of the North-East Atlantic.
"Battle Of Latakia": Fight as either the Israelis or the SYRIANS.
"Choking Halifax": Land SPETSNAZ forces and deploy mines from a SOVIET submarine.
"First Contact 1957 / 1973 / 1986 / 2016": Play as the Norwegians or the SOVIETS/RUSSIANS.
"North Pacific Shootout": Control either US Navy fighters or SOVIET PVO interceptors.
I could go on, but the point is clear.
