Edit: I should say I met him in a window where you can't get to him and he can't get to you and can only talk to him - at this point its the only time he's seen aside from the openning cutscene as far as I can tell. No, at this point he does not jump you and yes the bobbies and wellies just beat you up, they do not bring you to him.... yet.
It can be assumed as stated above since it shows him injecting joy and when off joy the wellies ask "sir are you alright, do you need to see a doctor/looks like someone needs to see a doctor." and the bobbies "Hows about you and me go for a little chat and see the doctor" that his main job is to inject joy and convert downers.
I think that'll become a threat later on, because instead of just injecting joy, you can overdose on it and "immune" people like arthur unless he's overdosed seem to only be immune to the joy's memory wipes - which seems to be it main reason for exisiting, something bad happened in wellington wells and everyone is trying to hide it. (I assume war related) So what if the doctor is not only the one who created the joy but is the one who when injects a downer - makes so he's fresh and not immune to the memory wipe anymore, which is why you get a game over cause the thing with the story is arthur is rebelling because joy is wearing off, and hes becoming a downer and is motivated to escape because he can see how horrible things are - if he forgets the truth perminetly by getting injected he'd just go back to work in the archives, and act like nothing happened at all! Game over!
Also it should be noted if you use joy around the garden distrcit poor people they will say "oh lucky you, joy works for you does it?" so it could be these people have taken it for so long that they no longer can even feel happy or "high" off of it like arthur still can, but there's so many of these downers they just tossed them in the garden district to get rid of them, which could explain why the doctor mains that gate I mentioned to save anyone worth saving from the Downer symdrome.
So as far as I know now, war happened and england (or at least wellington wells) is drugged. I suspect the doctor to be responsible for the joy
It's mentioned in one of the lores that joy doesn't work right for these folks - it tends to make the memories come back singed in flames, or something. So instead of getting them high, it makes them furious and makes them remember. That could all change when they roll out the actual story, though.
I imagine you will have more tangles with the doctor (and his Master or Assistants) in the future as you try to escape wellington well’s or end up in a different location, Say London or in main land Europe that may still have German troops where joy is being newly introduced. (Wellington was prolly one of a few test cities to see if the drug and there plan would work and that’s why the doctor personally over saw the operations. As I said just a theory.
