If you fail an encounter, thats it, no do overs?
Title asks the question. Its happened like 2 or 3 times now and I am just curious

I failed a quest and while I wasn't able to complete the "task" (tried a few times after I'd failed, only to be met with empty drawers/closets/etc) - I was able to buy the item from a merchant and reinitiate the quest and finish it that way. I was kinda bummed it's a one-and-done scenario on some of them.

Because the game is designed to be replayed, yes, certain quests are fully failable. This should not happen on quests needed to proceed, because we don't want you to be blocked. However, otherwise, we think a bit of permanent consequences for your action (provided the game isn't bugging out!) is cool. Makes everything just that little bit more serious.

That makes alot of sense. Some of those side quests though are super hard and to be one shot quests are intense. Just like The mansion you take joy at night, I tried to knock out one lady and failed because she detected me and on main quest i failed by accidentally killing a guy instead of giving him what he wanted (DARN V BUTTON!), but there is another way through the same gate so i just had to do the other way, which was a lot harder but I got it done.

Don't forget there are some quests you want to fail, or can fail to avoid doing stupid busywork. For instance I have never given faradays houseboy the valve, instead I just knock him out and steal his key. You technically fail a quest, but you get what the quest would of gave you anyway (Access to the annoyingly unfun Faraday Questline)
