Everyone says to see a doctor when you don't have Joy. Loading screen says he'll jab me with some.

So, I'm running out of Joy.

Walk up to a doctor.

Talk to him.

Wait.

Joy runs out.

Talk to him.

He sniffs the air.

Yeah, I’ve finished the game and replayed some of my saves a bit, never once been injected with joy by a doctor, they always go straight to drilling into me with their bonesaw.

If there's proof that doctors can try to give you Joy rather than the bonesaw, I'd like to see it. If there isn't, it'd make a great immersive mod. The doctor's character and motives would make more sense.

Alternately, like with Bobbies and bribery, there should be a button 'Request Joy'. Where...they give you joy.

There's a quest called "Edie Goes Downer", basically a doctor sniffs out that a lady is off her joy, so he takes out his chainsaw and starts chasing her along with an angry mob with weapons. Long story short, she's dead.

I think it's safe to assume that the image of doctors in this game isn't exactly what you'd imagine. In both appearance and behaviour.

You only need to take Joy twice in the entire game. In fact, blending is way easier without joy (no withdrawal)
My advice is to never take Joy in the first place

I've always wondered this myself. Plus, in the cinematic trailer, you see the bobbies beat down Arthur and then hold him down while a doctor tries to force-feed him with a joy. I wonder where this all went off to? Same with the stuff in the ABC of Happiness trailer...

But yeah. Doctors are nerfed crazy. It was a time you really feard them and did your best to avoid them.
Now you can just walk right by them off joy and .. meh. No need for sunshine or mojo.

I don't know how to insert the spoiler-function on this forum, but about these Doctors, you mean at the end of Arthur's act, right? The Coconut Joy?

What? They once actually did this elsewhere in the game but were nerfed? I also want to fear these doctors (even if I already do), the admins seriously need to buff the Doctors like in their next patch/update, especially since this thing is spoken off even in the game, even in the lading screen you get this notified.

Yeah, where arthur is captured and given the injection.

I honestly cant tell, but bobbys, doctors and wastrels where much more "agressive" in the alpha version then they are now.
Doctors now are to easy. Even in the quest going through the house you can sneak by them without sunshine or mojo. (If you have the fluid extrator thingy its quick to take them down)
