Legitimately i don't get it, is it a do over with extra stuff on top or just a different take altogether? i haven't played 1 or 2 so... should i just pay this or the next one...? are they going to keep doing this forever? wha?

Pathologic 2 is a reimagining/remake of the first game, which expands one of the game's three stories into a full game. They'd originally hoped to release the other two routes as DLCs later, but for various reasons the bachelor route (this one) ended up changing pretty considerably over the years, and at this point it's so divorced from both Pathologic 2's gameplay and Pathologic 1's original bachelor route that they're releasing it as a new entry instead. It's hard to say right now whether 3 is going to be standalone as an entry point or if it will rely on knowledge from Pathologic 2, but all you really need to know is that 1 is the original, and 2 and 3 are part of the same remake of it.

First game has 3 characters, 2nd game has 1 character, 3rd game has 1 character

So you can think of patho 2 and 3 as sort of remakes of the first game, but they flesh out the character differences a lot more.

In the first game, there wasn't a huge amount of difference between the routes (obviously there were some core changes, but generally speaking, they had a lot in common)

2nd and 3rd game are trying to make the different characters have unique gameplay mechanics

So unless you really feel like you want to experience the third character, you can safely skip the first game and go straight into patho 2/3
