Chloe is a lesbian. And she does not have a boyfriend, she had relations with boys in the past but that is not unusual for gay girls till they figure out their sexuality.
Its pretty obvious if you read her journal in "Before the Storm".
And her relationship with Rachel Amber was a romantic one, which you see if you play that game.

As for Max, she is 100% player determined, you CAN reproduce Chloes feelings depending on your choices but you dont have to.
But if you go for it its pretty obvious, Max talking about how much she loves Chloe in her diary and the final kiss at the lighthouse.

So in short:
Chloe = 100% lesbian, confirmed by the prequel game and other material
Max = player determined and can be straight or gay depending on player choice

It can happen twice, depending on player choices.
They both have boy friends, but not boyfriends. I think it's pretty clear that Max has no interest in Warren.
Also, read last page of Max's journal, that can make things more clear.

The kiss in the lighthouse scene (the pricefield ending), only happen, when you've always chose Chloe's side throughout the game amongst other things. For example not answering Kate's call, stand up for her against David etc.
In my playthrough Max just hugged her.

One of the things I really appreciated about DONTNOD's approach to Max and Chloe was the subtlety in the way they approached their character's personalities and relationships. Yes, it's fairly obvious that Chloe is lesbian early on, but you aren't beat over the head with it. It's just sort of there, as it would be in regular life. The way you potentially steer Max and Chloe down that path was just as subtle, and that was a pleasant surprise.

That said, choosing between Chloe and Warren? Not after you start to pay attention to Warren's character arc starting in the second episode.
