The problem this creates is it requires a huge overhaul of several systems. AI, AI reactions, combat for specific bosses that are meant to be one on one, and in game choices on conversational trees are only a few. Now you have rebalancing of how Exp and rewards are earned among multiple members as well as loot distribution. Since this is a FTP game then that is a huge issue for the developer and publisher as it affects players possibly visiting the game store, which directly affects money earned on the game itself. It would be great to experience but I don't see it happening soon.
What you just mentioned is one way to get around the problem. Possible new areas that haven't yet been coded could be a start. It still requires creating a new balancing system, but there is hope. There is a lot of land in Westeros or across the Narrow Sea. The thing they will likely look at is from where are they making the most profit on the game and that will undoubtedly get the Lion's share of developmental time.
Open World they can change later for a Coop Design.
The question is why they not set this up directly for the whole world.
